FOR ALL CLINICS

Individuals are encouraged to pre-register at https://h1n1vaccine.odh.ohio.gov/ to expedite their vaccination. Please complete and print out the registration form.

There will be no charge for these vaccinations. Vaccine will be distributed until exhausted.
